Landmark Health for Chronic Conditions

HealthTeam Advantage (HTA) partners with Landmark Health and its affiliated medical group (Landmark) to bring physician-led in-home medical care to our Medicare Advantage members living with multiple chronic conditions. Through the collaboration with Landmark Health, HealthTeam Advantage can now offer high-need members in-home medical services and care coordination, at no additional cost. Landmark services are offered to HTA members who have multiple complex chronic medical conditions (such as heart failure, chronic kidney disease, or malnutrition).

Our goal is to offer a program that lets us bring doctors, nurse practitioners, and other health professionals to our most vulnerable members’ homes to help keep them well and out of the emergency room and hospital. Helping you get the care you need, when you need it, is one of our guiding principles. Landmark provides front-door access to routine visits, urgent medical care, condition education, medication support, social support, and more.

HealthTeam Advantage recognizes the need for specialized care for patients with chronic conditions as they can have unique challenges (such as being homebound or home-limited) that make it difficult to see doctors as often as needed. Instead, these patients often go to emergency departments, which can lead to unnecessary hospital admissions and skilled nursing facility stays.

Landmark in-home care management is healthcare that comes to you. This program doesn’t replace your primary care doctor or other specialists–it supplements their care. It’s available any time (24/7) so you can get medical care and advice when your regular doctor’s office is closed. You can use Landmark for in-home medical, mental health, and social care; urgent care; routine exams; and hospital post-discharge visits.

As a HealthTeam Advantage member, you may be eligible for this program if you have a combination of any of the following chronic conditions:

  • Atrial fibrillation
  • Cancer
  • Cerebrovascular disease
  • Chronic kidney disease
  • Coronary heart disease
  • Diabetes
  • Disabling conditions (e.g., paralysis, amputation)
  • End-stage renal disease
  • Fluid and electrolyte disorders
  • Heart failure
  • Pressure ulcers with necrosis: stage 4
  • Protein-calorie malnutrition
  • Pulmonary disease
  • Severe chronic liver disease
  • Vascular disease

Landmark clinical services are available based on specific member characteristics that Landmark manages itself, on contract to HTA.  As such, the Plan cannot accommodate a member request for Landmark.
